Appointments System

URGENT APPOINTMENTS

Requests for urgent appointments should be made as soon as possible, ideally around 08.30.

The best way to request an urgent appointment is by phone - 01494 762323.

The triage doctor will assess the clinical need and determine when the patient needs to be seen, and by who.

ROUTINE APPOINTMENTS

All GP appointment requests are triaged in line with NHSE digital transformation plan.

We use eConsult for all GP appointment requests, this is our online appointment booking pathway.

To help us help you - provide as much information as possible about your problem, including symptoms and timelines, and attach photos where you think they may help with a diagnosis and/or treatment plan. 

Your request goes directly to the GP Inbox for triage. You will receive a response within 24 hours, although most responses are received within the hour. If an appointment is required, a member of the reception team will contact you to arrange this.

Some appointments such as phlebotomy, dressings, childhood immunisations,, smears and other Nurse appointments can be made either by phone or at reception during the reception desks opening hours, which are: 10:30 - 12:30 and 15:00 - 17:00 each day, Monday to Friday.

Cancel an Appointment

If you cannot keep your appointment please let us know as soon as possible to ensure the appointment does not go to waste and can be used by another patient, you can cancel via the NHS App - log-in --> navigate to 'Appointments' section --> select the appointment to cancel and follow the prompts.

Alternately, you can phone us and we will assist you to rebook accordingly. 

If you arrive late you may be asked to rebook.    

Patients will be asked to leave our list after 3 missed appointments, as it is considered an abuse of the system, wasting doctors and nurses valuable time, and it denies other needy patients that appointment.

Improved Access

Chesham & Little Chalfont Improved Access Service

Local GPs are working together to offer patients better access to appointments in General Practice in Buckinghamshire, known as the ‘Improved Access’ Service. 

This means that you can now book an appointment in a local practice up until 20:00 from Monday to Friday, on a Saturday and from 09:00 until 13:00 on Sunday at one of the three FedBucks Improved Access Hubs located across the county.

Patients registered with any GP Surgery within Amersham, Chesham and Little Chalfont will be able to access this service via the participating surgeries in their area.

Home Visits

If possible please try to telephone reception before 10am if you require a home visit.

A doctor or nurse may phone you back as your problem could may be suitable to be dealt with by telephone, or that it would be more appropriate to send a nurse, community service or indeed arrange a hospital attendance.

Home visits are only available for patients who are housebound because of illness or disability.

Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. There are also better facilities for examining and treating patients at the surgery.

Online services provide you with easy access to your medical records, you can see:

  • Allergies
  • Medications
  • Lab Test results
  • Immunisations
  • Problems (diagnosis)
  • Consultations and documents

You can also request repeat medication(s)

Online Services are provided by: 

NHS App- https://www.nhs.uk/nhs-app/

Available on both Google and IOS, no need complete application forms, simply self-verify via the app.

For patients with a 'smart' mobile phone, tablet or PC download the NHS App; this is available for both android phones and iphones.

You will need to log in and self-validate.

Once your account is active, contact the reception team (by phone any day after 10am) and they will activate your account.

Text Reminder Service

We have a texting service which allows you to receive confirmation and reminders about your appointments.

You will automatically be provided this service. If you wish to opt-out, please send a request to cancel the service via eConsult. 

Please remember to update your contact details with us when you change address, telephone numbers and email address.
